• Comprehensive Transitions Care - Ensuring safe, effective movement between hospital, adult care facilities, or home and community settings to help reduce avoidable admissions and readmissions, coordinate with the PCP, conduct member reassessments, develop and continuously update transition plan, transition back into the community, coordinate needed support services, medication reconciliation, and track admissions/discharges. HomeMeds/Medication Reconciliation - Reviewing and managing medications to prevent adverse drug events. 
• Recuperative Care & Respite Services - Helping patients secure and maintain stable housing.
• Short-Term Post-Hospitalization Housing - Providing temporary housing solutions during recovery. 
• Housing Tenancy & Sustaining Services - Helping patients secure and maintain stable housing. 
• Day Habilitation Programs - Supporting daily living skills and community integration. 
• Nursing Facility Transition - Assisting patients in moving from institutional care to independent living.
