We assure confidence that the patient has a thorough assessment to identify their needs and goals while having a guiding hand and assurance that they are not alone.

We develop a customized care plan tailored to each patient's unique requirements and preferences based on the assessment. 

• Provide 24/7 patient interaction, care, and a guided pathway to sustainable health and recovery
• Conduct ongoing assessments and follow-ups to ensure the patient is positively progressing and to support any new medical needs or concerns
• Assist the patient with scheduling and establishing a Primary Care Provider if one has not been established
• Assist with transportation to medical appointments if the patient does not have it available 
• Follow up with the patient to assure they were able to attend appointments without barriers.
•  Provide holistic care for optimal health and wellness

Upon final evaluation and determination that the member is healthy and no longer requires our services, we will finalize with the hospital’s case manager to close the case out.
