We develop a customized care plan tailored to each patient's unique requirements and preferences based on the assessment. 

• Provide 24/7 patient/member interaction, care, and a guided pathway to sustainable health and recovery
• Conduct ongoing assessments and follow-ups to ensure the patient is positively progressing and to support any new medical needs or concerns
• Transitional healthcare management to help patients/members navigate care transitions such as hospital to home
• Care coordination to ensure all providers are aligned on a single optimized care plan
• Assist the patient/member with scheduling and establishing a Primary Care Provider if one has not been established
• Health promotion and education to help patients/members build the knowledge and skills needed to manage their health successfully
• Referrals to community resources to ensure patients/members connect with social supports like housing, food, and employment assistance
• Assist with transportation to medical appointments if the patient/member does not have it available 
• Follow up with the patient/member to ensure they were able to attend appointments without barriers.
