Annually over $52.4 billion is spent to care for patients readmitted within 30 days after discharge for a previously treated condition. Timely outpatient follow-up after discharge has been a key component of contemporary efforts to promote better care coordination and address readmissions.

Assure confidence that upon discharge from the hospital the patient has a thorough assessment to identify their needs and goals while having a guiding hand and assurance that they are not alone.
Develop a customized care plan tailored to each patient's unique requirements and preferences based upon the assessment.
Provide 24/7 patient interaction and guidance to better care.
Conduct a 2-day, 7-day, 14-day and 21-day follow-up to ensure the patient is positively progressing and to confirm that they have no medical needs or concerns. Provide holistic care for optimal health.
Assist the patient with scheduling and assist with establishing a PCP if they do not have one.
Assist patient with transportation if patient does not have it available
Follow-up with patient to assure they were able to attend appointments without barriers.
Upon 30 days a follow-up will be made to reassess the member, if the member still faces challenges, services will be extended based upon the identified needs. This will be discussed with the hospital’s case manager to agree upon next steps.
Upon final evaluation and determination that the member is healthy and no longer requires our services, we will finalize with the hospital’s case manager for close out.

Ensuring each patient is connected with the correct specialized physician and first follow-ups are scheduled. We also assist with scheduling and ensuring the patient attends the appointment. If referrals are not developed, we ensure through the PCP that the patient is on the right path to have it done in a timely manner.
